ICD-10-CM 2027 changes
The FY2027 ICD-10-CM code set applies to discharges and dates of service from 1 October 2026. This page lists every change against the April 2026 update it replaces — computed from the official CMS files, not retyped from a press release.

Before you code a discharge on or after 1 October
- Use the release in force on the date of discharge (inpatient) or date of service (outpatient), not the date you code it. A patient discharged on 30 September is still coded with FY2026, even in November.
- 15 codes did not disappear but stopped being billable. CMS expanded them into more specific codes, so they still look familiar in your pick lists and templates — and a claim that uses them from 1 October is rejected. They are listed first below, with the codes that replace them.
- New category QA1 gives inherited cancer predisposition syndromes codes of their own: Lynch syndrome (QA1.71), familial cancer syndromes with pathogenic BRCA1 or BRCA2 mutations (QA1.790, QA1.791) and Li-Fraumeni syndrome (QA1.792). Until 30 September these were coded to Z15.09, genetic susceptibility to other malignant neoplasm, which remains billable for other cases.

How this page is built
Both code sets come from the CMS Code Descriptions in Tabular Order files (April 2026 update and FY2027), the same files behind the ICD-10 Code Browser. A code counts as new when it is billable in FY2027 and was not billable before; as expanded when it stays in the classification but loses its billable flag; as deleted when it is gone. Replacements combine the new codes that sit under the old one and the mappings in the NCHS FY2027 conversion table. The counts match the CMS FY2027 addenda: 190 codes added, 30 removed from the billable list.
